VISION USA™ provides basic eye health and vision care services
free of charge to uninsured, low-income people and their families.
VISION USA™ was established in 1991 by AOA doctors of optometry who
donate their services and it may be limited in
some areas. Open to children and adults. General eligibility requirements vary by state. COMPLETE THE APPLICATION FORM TO RECEIVE FREE SERVICES ONLY IF:
-You must have a job or live in a household where there is one working member full or part time.
-The person seeking care has no public (Medicare/Medicaid)or private insurance that covers eye exams (Medicaid does not cover eyeglasses in OK) -Have income below an established level based on household size and unable to pay for eye exams.
-The person has not had an eye exam in the last 2 years.
NO EXCEPTIONS WILL BE MADE
Your completed form will be reviewed to determine your eligibility. If
you are qualified and a volunteer doctor is available in your area, you
will be given his or her name to contact for an appointment.
You must answer all information and questions. Verification may be requested.
This form not to be used for application in Arizona, California,
Colorado, Hawaii, Kansas, Kentucky, Minnesota, Montana, North Dakota,
Wisconsin and Wyoming.
May be used for all family members. Applications are accepted year round.

Please allow 3-7 weeks for determination of eligibility for the VISION USA program.

comes in the mail with the name of an eye doctor. They cover the cost

Makes
referrals to ophthalmologists for eye examinations. Eligible seniors
age 65 or older receive a comprehensive medical eye exam and up to one
year of treatment for any condition diagnosed during the initial exam.
Eyeglasses, prescriptions, hospital services, and fees for other
medical professionals are not covered. Must meet income guidelines and
have no current eye doctor(ophthalmologist) or eye care insurance
through an HMO or the VA. Volunteer ophthalmologists accept Medicare
and/or other insurance reimbursement as payment in full. Patients
without insurance receive free care.

Vision clinic provides eye exams at no charge. Glasses are $25 for
adults; $10 for children. Mon., Tues., and Wed. at 6:00 p.m. By

InfantSEE®, optometrists will provide a one-time, comprehensive
eye assessment to infants in their first year of life, offering early
detection of potential eye and vision problems at no cost regardless of
income.

Sight for Students
operates nationally through a network of community partners to provide
free eye care to children of low-income working parents who do not
qualify for government assistance.
EYEGLASSES ASSISTANCE
New Eye Glasses for the Needy buys new for people in US and recycles to indigent countries. http://neweyesfortheneedy.com/ http://www.neweyesfortheneedy.org/ . Eligible clients can receive a voucher through a social service agency or a school nurse, or directly from our office. New Eyes for the Needy purchases new prescription eyeglasses for low-income individuals in the US through a voucher program. Clients can present a New Eyes voucher to any participating local optical dispenser, who fits the client with the proper prescription eyeglasses. The dispenser then receives reimbursement directly from New Eyes.
To be eligible for our program, applicants must:
- Be in financial need. Applicants should have incomes at or below the U.S.poverty guidelines:
- Have had a recent eye exam. (New Eyes does not pay for eye exams. If you need assistance in locating a source of free or low-cost eye exams,see info at top of page.
- Have no other resources available to them to pay for glasses,including federal or state programs or assistance from local charitable organizations.

Free eye exam and glasses for those who qualify One lady said she got her daughter a pair of glasses from here for $34 shipped because the ones she got at Wal-Mart for $150+ kept getting bent and the lenses kept falling out, etc. She felt like the glasses from zenni were much better and bendable so they won't break; she'd buy from them again in a heartbeat.

Provides
services for individuals with vision loss and blindness including

It’s possible to have diabetic eye disease and not know it. Many times
people with diabetes aren’t aware they have diabetic eye disease.
That’s because in the early stages there usually aren’t any symptoms.
So even people with good vision can suffer from diabetic eye disease.
